Duties/ Responsibilities:
- Conducts the admission process for new clients.
- Conducts initial assessments for new clients.
- Develops plan of care
- Conducts 90-day supervisory visits
- Verify competency of in-home caregivers.
- Assign appropriate staff to assist in-home services based on client need, staff qualification, and regulatory requirements.
- Understanding and ensuring adherence to the agreed plan of care for each client in your caseload.
- Partnering and communicating with all members of the client’s circle of care which may include family members, assigned Power of Attorney/spokesperson, community partners and related agencies.
- Monitoring and escalating changes in your clients plan of care, services, or condition.
- Conducting home visits and safety checks.
- Ensuring proper documentation and record-keeping for agency payers.
- Recruitment, screening, and selection of direct care staff based on Agency and external referrals.
- Determining schedules and adequate staffing and processing completed visits for payment.
- Provides initial and annual training and competency assessments for in-home care staff.
- Providing consultation to Direct Care Staff to ensure quality care and service Monitoring staff, identifying problem situations, and assisting in problem solving for optimum outcomes.
- Evaluating job performance of staff and providing performance reviews, corrective action, or other disciplinary/performance appraisals and counseling.
- Must be a registered nurse
- An individual who has at least two years of supervisory or management experience in home care or any other provider licensed pursuant to G.S. 131E or G.S. 122C; or
- An individual who holds a bachelor's degree in health, business or public administration science and has at least one year of supervisory or management experience in home care or other licensed health care program.
- Excellent customer service and communication skills.
- Solid experience working with the public, presenting a friendly, professional appearance while interacting with people.
- Ability to travel frequently within designated region or other locations as required.
- The major clinical responsibilities (initial intake and assessments, ongoing assessments) of the Supervising RN are compensated on a per‑diem basis.
- Client Intake & Service Setup | Performs initial intake and assessments and creates care plans ($160 per diem)
- Supervisory Visits | Re-assessments - Performs all required supervisory visits (every 90 days or more often) ($80 per diem)
- Care Plan Oversight | Writes, updates, and signs all care plans ($65 per diem)
- Other administration work on per hour basis. (Range $40 - $50 per hour based on work allotted - trainings, competency testing, policy reviews, etc)
