Job Summary
The Registered Nurse (RN) plans, organizes, and directs home care services for the agency, drawing on professional nursing experience and community health knowledge to plan and deliver care that meets the needs of individuals and families in their homes. The RN is responsible for the development, review, and oversight of individualized care plans for clients receiving non-medical home care, supervisory home visits, caregiver training and competency evaluation, and quality assurance. The RN ensures that all services provided are consistent with each client's needs and adhere to North Carolina regulations, agency policy, and the State Nurse Practice Act.
Key Responsibilities
- Complete initial in-home client evaluations and develop person-centered plans of care.
- Review and update plans of care at least every 90 days, or sooner as client needs change.
- Perform supervisory visits to clients’ homes to assess caregiver performance and ensure care plan compliance.
- Provide training, in-service education, and competency verification for in-home aides.
- Document assessments, supervisory visits, and care plan changes accurately and timely.
- Communicate with clients, families, and agency staff to coordinate and adjust care as needed.
- Ensure compliance with all applicable regulations and standards of practice.
- Participate in quality assurance activities and assist with audits or surveys.
Qualifications
- Active North Carolina RN license in good standing (or compact-state license valid in NC).
- Minimum of 1–2 years of nursing experience (home care or geriatric care preferred)
- Strong assessment and documentation skills
- Excellent communication and interpersonal skills
- Ability to supervise and support non-clinical staff
- Detail-oriented with strong organizational and time management skills
- Valid driver’s license and reliable transportation for home visits
- Familiarity with state home care regulations and best practices

Compensation: $40.00 - $50.00 per hour
